What is the SF-36®?
This comprehensive short-form
with only 36 questions yields an 8-scale health profile as well as summary measures
of health-related quality of life. As documented in more than 2000 publications,
the SF-36® has proven useful in monitoring general and specific populations,
comparing the burden of different diseases, differentiating the health benefits
produced by different treatments, and in screening individual patients.
What is the SF-36v2?
This manual was prepared to be a companion to existing manuals about the SF-36®
Health Survey. This manual documents the improvements in SF-36®, Version
2. This manual also presents up-to-date norms for SF-36® scales and summary
measures as well as citations for nearly 1,500 publications.
What is the SF-12®?
An even shorter - 1-page, 2-minute - survey form has been shown to yield summary
physical and mental health outcome scores that are interchangeable with those
from the SF-36® in both general and specific populations. This short-form
- the SF-12® - which was published in early 1995 is already one of the most
widely used surveys.
What is the SF-8?
The SF-8 Health Survey represents a major advance in the application of
SF technology for purposes of achieving both brevity and comprehensiveness in
population health surveys. It has only eight questionnaire items. By relying
on a single item to measure each of the eight domains of health in the SF-36®
Health Survey, the SF-8 represents the ultimate phase in the evolution
of SF assessments - single-item scales.
SF-36® Norm Based Scoring
(NBS)
The new Norm-based scoring (NBS) for the SF-12® and SF-36® is a recalibration
of the original SF-36® and makes the profiles easier to interpret and results
directly comparable with the widely used summary measures.

The SF-36 has become one of the most widely used generic measures of subjective
health status. However, despite the widespread use of the measure criticism
has been forthcoming concerning the layout and wording of some of the items.
Consequently, the developers have produced a modified instrument, the SF-36
Mark 2.0 (SF-36II), which is a direct descendent of the SF-36 Developmental
Form and the SF-36 Mark 1 Standard Form. The SFII was included in the Third
Oxford Health and Lifestyles Survey (OHLSIII). This study introduces the UK
SF36II and assesses the internal consistency reliability and construct validity
of the measure, and the physical and mental health summary scores. A paper is
in press with the Journal of Epidemiology and Community Health.
